For Initial Consultations please call either Dr Mandy or Dr Lisa directly and they will advise which forms to complete ahead of your appointment. These forms are interactive if you have pdf writable software and can be emailed to your practitioner ahead of your appointment, otherwise please print out, complete and bring with you to your appointment:
Fees
Initial Consultation (including report fee): $130 / $115
Standard Consultation: $65 / $60
Examination: $25 + Standard Consultation
(Please note the lower fee is available to concession card holders and children under the age of 18).
Cancellation Policy
We are a small boutique business and do our best to accommodate all of our valued practice members. We require as much notice as possible if you are unable to make an appointment as there is a wait list for appointments on any given day, so we appreciate your consideration.
Our cancellation policy is we require 24 hours notice, otherwise a $30 cancellation fee is incurred.
Full fee payment will be required for a missed appointment with no notice.
